A woman's nipple's will become hard/erect when she is aroused - when you or she has done something to her body that has made her body respond.

But it usually is NOT just because she is horny or thinking she wants sex. It takes more than that.

Yes, but hard nipples do not mean a girl is horny. If means she is either cold - or is aroused sexually. Being aroused is different than being horny. Being horney means you want sex - being aroused means you have seen or experienced something that you found sexually exciting and you MAY OR MAY NOT want to have sex because of it.
