how do you tell when a torn frenulum is too serious to heal on it's own or if it needs stitches? If it does, how much would the procedure cost without insurace? (ballpark guess)

I'd say that if in three days time you don't see signs of healing - you need a doctor. What it will cost will depend upon your pain threshold - if you need anethesia it will cost more depending on what type. Could costvery little - like the cost ofan office visist.
