I would not do an anonymous call. You don't know if the nursing home wants to defend themselves, than they may asked to listen to the message that caused them in this trouble. I would send several letters to the State and use different names and addresses, if you can, and type the letter out so your writing will not be traced back to you. While trying to complain about your NH, you should protect yourself as well.
