the staffing will never be enough! it is so hypocritical of the ltcs' to state in their mission statements that their goal is to provide the best quality patient care when we all know it is not true. there is a huge "don't ask, don't tell" mentality. administration doesn't really want to know how bad it is because then they would have to do something about it. so they pretend that there aren't shortages of workers, supplies, linens and towels, etc. a nursing administrator will come whizzing through and comment on whether the patient has the correct shoes on when you are busy trying to locate towels from the laundry so you can clean 8 or 9 other patients that are still in bed waiting for cares. the person who said that administration should shadow us on our daily cares is so right. it should be a requirement--one day a year!! see what goes on--try to keep up!! it is a thankless job and cna's are considered very dispensable. i am glad that the federal government, as of january 2010, will not reimburse ltc facilities under medicare/medicaid for any bedsores generated at that facility. already my facility is scurrying around trying to educate everyone on wound care. i've seen creams and pads available that we could hardly get our hands on before. but unltimately, they will have to address the staffing, because overworked cna's can't possibly do the screening and cares necessary to prevent bedsores adequately. it's a shame. i love my residents and get very frustrated to see them getting short changed. subacute is the only unit adequately staffed because that is where the facility makes money. our ceo said that they lose 20 cents on the dollar for care given to the long term care residents.
