Where To Get Good Cut & Paste E-Mail Form?

Hello everyone,

Many thanks to those who gave me advice re: PDF downloads!

I plan to launch a modest newsletter from my site once my e-book goes "live" in about 10 days.

I am looking for a nice, clean, cut and paste HTML form that I can put up on my site to collect Name and E-Mail addresses of "opt in" visitors.

Can anyone point me in the right direction on this?

As always, any advice on this would be very much appreciated.

Use http://www.informationbyemail.com

This is an auto responder that can also be used as a regular mail server. Very easy to use, and it works great. I use it to give away my "37 Secret Money Making Strategies".

the way I do it...

Well, the way I do it, is I use a free form from http://www.responders.com.

I can put whatever I want on the form, and ask up to 10 questions if I wish. After someone submits the form, it takes them to a thank you page that I designed, then that person gets an email from the built in autoresponder.

Their information is then automatically emailed to me. I look over their info, and put their name and email address into my ezine.

I get paid to place ads, not to sell things.
