It's caused by the stiffness in the rubberized necklace...

Hi Matt,

The rubberized necklace isn't very flexible and tends to flatten out. The loop opens up and the ring can slide out. I've had this happen once when I switched over to the rubberized version and the first night sleeping found out that the ring came off when I woke up. It took me a while to figure it out.

BTW - There's a intertwined string/rope type necklace from AP that is more secure but it's not really water proof:
