Why?

Well this year i have lost more fish to the same symptoms (redness and ulcers that may appear anywhere on the koi).No flashing is seem.Last year deaths were nearly the same, and i seem to notice the red marks on the fishes after the 'hose pipe' is used to do a clean up or a water change.I do used deco, but only add it into the pond with the hose pipe going in, is this the correct way?I have tested the water every time and all is ok, ph stable to 7.6, Ammonia 0.0,Nitrite 0.0. Salted before to 0.3 in all ponds for 2 weeks and then 10-20% water change every weekend.Have used medifix in a q- tank but the fishes just lasted 2-3 days, it seem the bigger fishes are dieding, the goldfishes are not showing signs of illnesses, even my last years baby frys are all ok.
My chakoi is bad with tail rot, what i thought was carp pox, turn out to be tail rot, and i have treated this with iodine and anaesthetic spray.Now its going worse, only yesterday i netted the fish for treatment and it bleed from the gills which is normal, but after treatment it was very stress out and was hanging about near the surface for over 2 hours, today it seem fine.Its very down hearting with all these fish lost's.The next fish that dieds i will scrape and micoscope plus open it up to view organs.just didn't have the bottle last time with the fishes being a 'pet'.

But why is it alway when i do a water change i notices the problem to hand?
ps, i hold my hands up, i have problems and i am going round in circles trying to sort this out.
