HERE'S WHAT HAPPENED...

Response to OK ...lol

I went to the school and pick up a blue immunization card. Interestingly the back of the card reads 'This form is not to be sent home or given to parents to complete.' The next morning I gave the form to his teacher, along with the yellow form that I was suppose to take to the doctor and have signed after immunizations were given. When I picked my son up this afternoon his teacher informed me that the office would like to see me about the immunization form. I went to the office and the person there separated the blue card and placed it on the side, then she pushed the yellow sheet in front of me and said, "I don't know what the school nurse told you, but this isn't what we need." (pushing the blue card away) She then pointed to the yellow sheet and said, "What WE need is these immunizations (circled) to be done by Friday." I then reached over and grabbed the blue card and turned it over and said, "Yes, I know, and I signed here." Her reaction told me that she knew the card was signed. So I said, "I guess we're all set.", and she gave me a very cold "yes" reply. I thanked her and walked out. She was right behind me a minute later calling out for me to come back. She started to say, "Do you know...", I stopped her and said something along the line of being fully aware of what my signature meant and what my rights are under California law regarding immunizations. That seemed to stop her in her tracks and she said "okay".

Many thanks to all that replied.
