First off, I just sorta glanced through the rules and they sound plausible to me.
Some initial thoughts .... First off, do the WPs from areas like Sewers in Act2 and Halls of the Dead need to be found ? They are completely unnecessary for progress (not a huge deal either way).
Acts 1 and 3 will have several situations where an automatic heal will happen, since you get quest rewards from Akara and Ormus. Allow it or gulp a pot prior to visiting ?
Since all items will break rather quickly, if there's an Am in the party I suggest imbuing bows only as they will never break, by all party members until a good imbue is reached. IMO, imbuing melee weapons is completely out due to the fact that they will break within 10 minutes of fighting. Unless of course, you want to adjust the rules and say .... make weapons or all items or maybe 1 item of choice repairable at an end of an Act or something like that. This will at least give some meaning to all non-bow imbues.
Should the Cube quest be repeated in the higher difficulties ?
That's it off the top of my head ....

ps. IMO, without any repairs at all, melee characters will become close to useless in anything but Normal, maybe even a bit before. Maybe the *once/Act* repair of weapons is a good idea ?
