b2600i runs VERY rich

I have a 91 b2600i with 113000 miles. Just had heads worked on b/c of oil problem. Now that that's finished, I realized I also have a rich fuel mixture problem.

what are the possible culprits? I know of the engine coolant sensor, fuel pressure regulator, o2 sensors.

anything else? I'm not getting a check engine light, but my plugs are fouled big time. Black smoke and difficulty cranking it.

can the MAF cause a rich fuel mixture?
