I think it's an excellent idea. Several times each year I provide written evaluations for my goalkeepers (even the u-12's). It's important that the kids understand the purpose of an evaluation- hopefully to provide them with some feedback and ideas to improve their game. I typically critique their current performance, and include drills and exercises to help them develop as a player.
Areas that you might cover:
-Ball control and touch.
-Positioning and movement off the ball.
-Strength, speed, stamina, and agility.
-Crossing, passing and shooting skills.
