It's fairly straightforward. There's an explanation if you click on the little ? button on any of the voting boxes. Here's a synopsis:

People can vote "Yes", "No", or "Maybe".

The Otaku Approval number is equal to the total number of "Yes" votes plus half the number of "Maybe" votes. For example, if a doll has 10 "Yes" votes and 10 "Maybe" votes, the Otaku Approval number will be 15. The number of "No" votes has no effect on the Otaku Approval number.

The percentage is the Otaku Approval number divided by the total number of all votes. For example, if the previous example also had 10 "No" votes, that would make a total of 30 votes and 15 divided by 30 is 50%.
