Child with special needs, not learning is public school.

I have a 12 yo daughter who has multiple disabilities. She has various LD, plus seizure disorder, ADHD, PDD-Nos, MR, OCD. Her behavior is problematic in school. She is going into 7th grade and works on 1st to 3rd grade level. I am wanting to homeschool her, mainly because she is not learning in regular school, even though she is in special ed. and school is too much of a hassle-she is always in trouble.

Previously I never even considered homeschooling, but now it seems the only way to go. I don't know where to start.
