I feel "hay fever" to be a fixed term. That's why I consider it to be quasi a proper name and I don't analyse it gramatically, I just take it as if it was one word. But I am still undecided about this ... To translate it more word-by-word would also be a good possibility.
Yes, it would be the most natural Slavic to have -it --> -en'e, but I don't know how the verbal nouns from verbs in -et would end, then. That's why I have -t --> -n'e.
