I was just reprinting the form for the trachea study and it says:
"To receive your reimbursement (up to $50 per dog for the x-ray procedure) you must return this completed form, along with a copy
of the receipt for the x-ray, as directed at the bottom of the form.
It does not say anywhere on the form that you have to send a copy of the paper from OFA. I do have the form, but I think something needs to be added to the BCA form.
