I have put one up online, that you may run off and send in, for the non qualifiers divisions! Those that had to qualify needed to get one at their districts.
Location on line is the State USA website that I do, even if you open the flap USA site you can bounce over with the link and get to the one I manage. Anyway it is up on there.
hope you can find it.
