swollen feline pads

i have a 15 yr old cat. dx w/ renal failure 3 yrs ago but doing very well- her renal functions has not deteriorated during this time.
she has developed swollen pads of the 3rd digit (symmetrical) of both rear paws. she has been on a 10 day course of baytril, 3 wks of amoxicillin, & 2 weeks on prednisolone all w/ no change so far. her claws won't retract so the area is being cleaned w/betadine 2x daily. x rays showed mild arthritic changes on those to digits only, no gas pockets. my vets are very stumped as to what is causing this. we are reluctant to biopsy due to the renal issue.
has anyone heard of anything like this before?
please help!!
