I have just change status from F1 to F2 visa aproximately 45 days ago. Now I would like to apply for work authorization due to the high dental and medical reason of mine and my husbands. I would like to know what form do I have to submit to INS and what kind of information do I have to attached to the form.

With an F-2 visa you cannot obtain work authorization. Your husband, however, can apply for off-campus employment based on severe economic hardship if he has been in F-1 status for one full academic year.

His application should include documents that show that he has encountered "severe economic hardship caused by unforeseen circumstances beyond [his] control.” Such documentation should include copies of medical records and medical bills if his request is based on medical hardship.
