Hi there!

Im new to this website.
Id appreciate it if you could take your time and read about my problems and answer my questions!

I realized that most of you are highly experienced in the POF / DOF / high FSH area.
I have a question so Ill give you a bit of background about my problem.

Ive been ttc almost 5 years. I had a bit of endo but that was excised during my 2nd lap (1st was a bad experience which didnt help at all!)
We always thought I had a blocked tube because of a HSG but the 2nd lap showed that both tubes are absolutely ok.
When we were doing natural injectibles cycles, the doctors realized I was a bad responder, which led to a FSH test that was 18.9. After that it went down to the 10's, up to 24 and 25 and keeps going down to 7, 8, then 16, 11..... a real mess.
By both of my laps, the doctors noticed that my ovaries are smaller than usual, and not as "smooth" (lack of a better word) as it should be.
So we "ran" to IVF.
I have done IVF 6 times in a bit over a year.
Twice, the follicles were empty.
Once, an egg was retrieved but had to go to the freezer because of my lining.
Once we transfered 1 day-2 embryo but no pg.
Once we transfered 3 day-2 embryos and I had a chemical pregnancy.
And 2 weeks ago we transfered 2 day-2 embryos and the thawed one - but no pg...

As Im having my follow-up consult with my RE, I decided I want to review ALL the options.

So my questions are:

1) Do you think IUI would be good in my case? Better than IVF? (Sounds weird, but..)

2) The best protocol we have found that works for me is Femara 5mg + Gonal 150UI + (sometimes) Menopur after I stop Femara. Ive made between 2 follicles (1 mature only) and 4 (all mature). Do you think a change in the protocol would be good, here? If so, what would be good?

3) Do you guys also do day-2 transfers when you have 1, 2 or 3 fertilized embryos? What's your take on it?

Thanks so much!!!

I just wanted to say that your protocol of Femara and injectibles is exactly what I'm doing. I got 5 eggs for my IVF, 3 mature, 1 borderline and 1 immature. 2 fertilized and ended up with an 8 cell and 6 cell embies...but BFN. I did a day 3 transfer. I've gone back to IUI's because I can't afford another IVF and did the same protocol this month and had 5 follicles that were a big bigger than my IVF follies at trigger....so I hope I had 4 mature......one was smaller like around 13mm....so immature. The others were 23 and 3@21, so that was great for me. I've done almost every protocol including microdose lupron with moderate stims and on straight injectibles I only get one follie....so if the Femara works.....stick with it! Just my opinion because my body loves it. Here is a great site to check out for general info. http://www.highfshinfo.com/

Keep us posted on your next steps.

xoxo, Tracy

Thanks for your reply!
How interesting that we both use the same protocol.. is your RE one of those who work "specially" with high fSH cases?
Have you ever done IUI before or is it the 1st time?
Also, what meds did you use after transfer? Was it only progesterone or also Estradiol?

BTW I once used Lupron before I started stims and it totally SHUT DOWN my body. Nothing happened. I HATE lupron with all my heart.

anyway thanks again!

I'm with...

Dr. Danzer at Southern California Reproductive Center in Beverly Hills. Where are you going? Interesting that we are on such a similar protocol. After my IVF Transfer I did PIO shots and Estrace. I also did Medrol for 4 days starting at retrieval day. I've done a TON of IUI's. I'm single and using frozen donor sperm....so my RE thought it would probably take at least 6......then I started testing high on the FSH and having low responses....so it is a double whammy!! Oh and I'm 43!! I was soooo hoping IVF would do the trick. But, I'm not quite done yet with the IUI's. Still praying for a miracle BFP in the next couple of days......if not, I'll be into another IUI cycle right away if no cysts on CD 2.

Im in South America.. wow!! 6 IUI's.. its a lot! well, ive done 6 IVF's and nothing...
How are your Estradiol levels in the 2ww? Cuz mine go really high because of the Estradiol I take, and then next cycle the Estradiol never drops so I always have to take BCP for a whole month and only then, the next cycle start another cycle. How anoying!! Does that happen to you too??

Why do you take that other med for 4 days? I wanna gather as much info as possible b4 I speak to my RE. Any other tips?

Thanks!

My clinic....

July 24 2008, 10:36 AM

uses the Medrol (a steroid) as a standard protocol before embryo transfer. It is supposed to help with possible embryo rejection. My E2 levels are usually fine in the luteal phase. I haven't had a problem with them dropping back down when AF shows. I did find that cycles after using Estrace in the luteal phase I was somewhat suppressed. I make more follies without using BCP's or Estrace the cycle prior. Good luck at your consult!
