Flat-line question

When treating a patient in the field and a patient's heart stops (heart attack) after first exhibiting irregular heartbeat and bradycardia, then goes flat-line after defibrillation, what is the procedure for administering a drug directly into the patient's heart? Is the drug epinephrine or something else? What size is the dosage for an adult male?

A shot a the answer

Well... I'll take a shot. First, your not going to see drugs injected directly into the heart in field. Second, you do not shock a brady rhythm (pardon the spelling if I goofed). Now assuming you head an irregular heart rate that bradyed down and went into either pulseless v-tach or v-fib then the first line of drugs is typically epi 1mg, followed by a defib, then either lidocaine at 1.5mg/kg or 300mg Amiodarone HCL. All these would be IV. The amiodarone is relatively new. Which drug is used depends on local protocals.

Flat-line answer

Intracardiac medication is rarely, if ever used. And that was only in the hospital back in the old ('70s) days. The movie Pulp Fiction was just that, fiction. If a patient that was in cardiac arrest (BTW, heart attack usually refers to a myocardial infarction, not cardiac arrest) and in ventricular fibrillation (or pulseless v-tach) was defibbed into asystole (flatline), the medics would start with the asystole protocol which is usually (following intubation and IV access) 1 mg IV of 1:10,000 epinephrine (1 mg in 10cc), followed by 30 secs/1 minute of CPR to circulate it. Then follow that with 1 mg IV of atropine and CPR. After 3 to 5 mins, repeat the medications. Treatable causes (like hyperkalemia, acidosis, etc.) of asystole are considered too, but in most cases, asystole is usually irreversible. We are rarely able to resuscitate asystolic patients.

Erik, I guess I have a (maybe morbid) curiosity about it. I had surgery last year and was given a little Demerol to control some shivering I was experiencing in post-op (I had had a spinal). At the time the shivering decreased and I dozed a little. Later on, as the spinal wore off, I began to experience some pain. They didn't want to start me on MS until I was in my room, so the nurse gave me a little more Demerol. When she did, I felt like a flash-fire was going through my veins and felt suddenly very nauseous. I told her I thought I was going to be sick and she told me to take deep breaths, that it was the Demerol. I remember taking one deep breath and squeezing my eyes shut, then nothing. When I woke up again there were about 8 people leaning over me, calling my name, and holding an oxygen mask over my face. A few minutes later my anesthesiologist came in and was talking to the nurse about what happened. She said, "She brady'd down to 40, and then this." She showed him the strip from the monitor, he looked at it, then said, "15 seconds with no heart rate." Then he checked it again, and said something like, "When you paged Dr. ????, he told you to give her 10 mg. atropine?" She acknowledged this, but said she only gave me 5 mg because my heart started beating again. He then said something to the effect that she should have given me the full dosage anyway.

Obviously, I had had a reaction to the Demerol, but I was just wondering what the difference would be in the field. Naturally, my problem wasn't an infarction, but this stuff fascinates me.'

Actually, IC epi was a field treatment in the "old days", at least in my state. This was before they decided that epi could go down the ET tube. So if you couldn't establish an IV or the patient didn't respond to the IV meds, we would go IC (with orders, of course).

Okay, I'm looking for this story I read awhile back. I've asked others if they knew where it is but it seems everyone remembers the story but not the site it was on.
So, now I'm asking you. The story is about John getting raped. No not ASCENT and DESCENT. This story is about being missing showing up in the hospital, Dix gives him a teddy bear, and his attacker kills himself.

Fan Fiction search for author

I realize that this answer is being sent 6 months after your request, but maybe it is a case of better late than never. I think the story you are looking for was posted on Squad 51, Where Are You? It was written by Rebecca B.and was entitled "Victim" I copied many stories onto my hard drive when SQ51WRU announced that it was shutting down way back when. I think I may have an email address for writer.

If a patient is very distraught following an assault that includes a head injury, to the point that the doctor can barely treat him, can any sedatives be given? If no sedatives can be given, what measures can be used? Restraints? By the way, the patient is distraught because of the assault, not as a result of the head injury. Thanks!

John Smith as Capt. Hammer

I just watched the episode titled "Hang Up" on TVLand (6/28/01) and noticed (for the first time in MANY years) the company captain is not Dick Hammer. BUT, the credits list him as John Smith AS Captain Hammer. Whats the deal? Do you have any info regarding John Smith? Where was the REAL Dick Hammer. Little notice is given to this mystery Captain. Just Curious.

Dick Hammer (who was a real LACoFD Captain and had a SAG card) quit the show because it started to effect his job at the LACoFD. This was due to the amount of time on the set and trying to get coverage for his shifts. I believe "John Smith" was used on the show as the staff tried to find a replacement for Dick Hammer.

From What I understand Capt. Hammer was an actual member of the L.A.C.O.F.D. and ultimitley left 'EMERGENCY' due to scheduling conflicts between his duty shifts with the department and 'EMERGENCY' Scenes. Dick Hammer was an actual Captain so I'm Sure that added to his duties with the LACOFD.

A delicate question

A ruptured testicle is certainly a possibility - honest! Bruising and swelling of the groin area is also a possibility. Treatment/transport in my area might consist of: placing the patient in a position of comfort, placing a cold pack on the affected area, inspecting the injured area only if severe swelling is suspected (usually we ask the patient to take a look first - it's quite embarassing for them!) Our paramedic's may consider starting an IV of normal saline depending upon the severity of the injury. More likely, they may just establish a saline lock.

An experience I had

Fellow emergency fans, I had the best experience and I could not help but share!

I was on our engine 1, and we were pullin up into the mall parking lot. As we turned the corner on the hill into the parking lot I saw it. It was a Crown Firecoach! I was happy just to be near one, but as we parked our truck and I got out to talk to the chief and he gave me a list of activities.

It was a normal list, the good safety fair stuff. A confined space rescue, a car extrication, land STAT Medevac, go up and do an aerial demonstration...But then I saw it! It said demonstration with privately owned engines. The Firecoach was one!

So we went to the station down the street and they set the siren off and we rolled. I had just my helmet on and everything when we rolled up in the Firecoach. We then pulled 2 of the lines and squirted some water. This was the best safety fair I've ever been to!
