Diane has this whole synapsis before she pulls the trigger on herself. The old people represent her sanity, which she has still made peace with in her fantasy land, but when reality strikes her again, they come after her showing she has gone nuts, followed by her taking her life. The two guys in the diner our the detectives investigating the death of Camilla. The "mud woman" as ignorantly put, is not a woman, or a man for that matter. It is death. In fantasy land Diane has the detective go out and seek him, and guess what happens to him. He see's death, he dies. The fantasy simply represents how Diane wishes life was. She actually believes this is reality until her and Camilla our at the club where I think she starts to realize this is not real, thus the uncontrolled body spasms. The blue box and the key. For one the key Camilla had and the one the hit man gave to Diane our completely different kinds of keys and I believe our unrelated. The box represents Camilla's memory. Her key will unlock her memory, and when they goto the club her memory starts coming back to her hence the arrival of the box. As she gets the key to unlock the box, Diane disappers. This is because that Diane never existed.......I have more ideas but it is really late. But the thing I haven't figured out is the cowboy.
