I think Don Simmons offers dual exhuast pipes for C-cab trucks. He essentially brings the left side pipe over to the right (like the stock single exhaust), but runs the two pipes together back beyond the fuel tank. He then split them apart again to exhaust separately out the back. Not sure his pipes would allow them to come out in front of the rear wheels.
