I am not naive nor egotigistal enough to compare one knowledge base to the other because the bottom line is patient care within the context of the scope of practice, and all practitioners should be big enough to acknowledge their limits. (But if you want to know, all of the OD's I shadowed worked in clinics and hospitals.) As far as the setting of patient care, the bottom line is as long as the patient is happy, why should the non-patient care?
