"allow the nurses, PT, and others more opportunity in treating rountine/palliative foot conditions." I had one housecall today and 2 patient treatments (my gross was $110) and I don't even practice anywhere near NYC (where 20" of snow just fell).

We let (or didn't stop) the PTs and DCs from prescribing & dispensing orthotics. I was referring patients which would benefit from physiotherapy to a local PT in my area and told them to come back for orthotics. The PT found out what my rate for orthotics was ($450) and undercut my fee by charging $350. So now when patients, which can benefit from physiotherapy & orthotics, come into the clinic, I cast and dispense orthotics before I send them to a PT. As for RNs doing foot care, more and more are doing basic/routine foot care in my area and business has been decreasing for me for the past 5 years. The MDs won't let DPMs become podiatric-PHYSICIANS!!!!!! They'd rather see DPMs perform basic, routine conservative foot care. I've even heard of an orthopod who has specialized in orthotic therapy. He claims that he'd rather cast patients (with foot pain) and dispense orthotics to avoid/delay surgery. When that doesn't work then he does foot surgery. Depending on the surgery/results, I'm sure he has the patient get another pair of orthotics. Based on my own experiences, I have to vehemently disagree with your suggestions.
