With regard to the narrow foreskin, and the matter of "cleaning" it, this has already been pretty well dealt with in this thread
over the last week.
Now, you seem to be describing an attachment from your glans to the foreskin above
the urethral meatus ("pee hole") as distinct from the frænulum below it. If so I suspect you are describing a "bridge" under which when you pull the foreskin back to see it, you could thread something (such as a knitting needle) from side to side, unlike
the frænulum which is a complete "web" - you cannot get anything under it (though some people do pierce through the middle of it and hang decorations!
If this is the case and it is a narrow "bridge" or "adhesion", you might actually be able with a little daring to pull it loose from the glans (penis head) - it would leave a raw area but that would heal; Zinc and Castor Oil (something to which you will see me refer frequently in this forum) would be soothing on such a raw area.
Again, do a bit of heavy reading here to understand the dynamics of stretching in general.
