You pull the foreskin away from the glans (penis head) by "pinching" on each side with thumb and forefinger of each hand so that it forms the "funnel" you illustrate where the opening of the funnel (your "part A") is the part of the foreskin that when you pull it back, will come over the penis.
The actual tight area - your "part B" - normally resides deeper inside the foreskin, immediately in front of the glans and it is only by pulling the foreskin forward that you will be able to get another finger down the "funnel" and in through the opening "B" so that the finger tip can sit inside, just beside the glans (which may be sensitive, so that finger needs to be lubricated).
Once you are able to get one finger through - which we realise is quite a tricky task due to the need to use both hands already to create the "funnel", you work towards getting the largest finger possible through, and then move on to two fingers. It's a skill you need to develop and you will get smarter and smarter at it so that eventually it should be quite quick to get two fingers (one from each hand) inside and beside the glans, letting go of the "funnel" and levering those two fingers apart to effect the stretching.

Opening B is very small. I cannot get a finger through it
With some lubrication and the nail trimmed smooth?
If not, you will simply have to start with a smaller object such as the smoothly rounded, "torpedo-shaped" plastic cap of a ballpoint pen.
The point that I wished to stress as the most critically important, is that you simply will not be able to get anything in the hole by pushing the foreskin against the glans, the only way is by pulling the foreskin forward on both sides (just as you need to hold a shopping bag open with both handles, in order to put anything inside) and pulling it away from the glans, forward over the finger or other object.
