| I went to the gynecologists for the first time on Wednesday.He did the usual breast exam, vaginal exam/pap smear. Afterwards he said everything looked okay and I didn't have to come back for a year. He justed needed to get results of pap. But now I am confused/worried because I took a look at the paperwork of procedures/conditions/diagnosis before giving to receptionists and looked over my copy and he had three diagnosis circled but did not mention any of them to me. One was cervicitis which could be a possibility I guess. but the other two were abnormal uterine bleeding and dysmenorrhea(severe menstrual cramps/pain). I never mentioned those two to him as problems I'm having nor do I bleed heavily during my cycles. My menstrual cramps are not severe to where I can't move or do anything during that week. They are actually minor and I always go bout my week as usual and no one even knows when Im on my cycle. So I'm really trying to figure out where these diagnosis came from and if they are correct. Especially since he didnt mention any of this to me. So do doctors always write down conditions or diagnosis without telling you about it? Or is it possible for him to be waiting on the pap results before he discuss the other diagnosis with me? Or do they just have to have something filled out on the patient forms for insurance purposes/payment? Because right now I just dont understand how he could tell me everything is fine and to come back next year but then write down those three diagnosis. I know that the abnormal utering bleeding and severe menstrual pains are not a real issue for me but I am worried about the cervicitis. I know I couldn't have gotten that infection from any type of STD becuase my husband and I are both each others first and i dont use tampons or douche. I read that it can also be caused by injury to the cervix. Im thinking thats more of the reason if that was a true diagnosis by the doctor. Sometimes my husband cant find the right spot and on several occassions jugged to hard with his penis or fingers while trying to get in.Im guessing that he may have been causing injury to my cervix all these times.Most of the time I can feel that he is in the wrong area and feel funny afterwards. So is this a possibility? Because that is the only thing I can think of because none of the other causes that I read about are possible.
Please help ease my mind if possible.

I am a medical biller and have worked for OB/GYNs for almost 20 years. Your diagnosis on your statement, although incorrect, was just as you suspect for insurance purposes. Most insurance companies will not pay for a routine yearly GYN visit when no problems exists. Although your doc could get into trouble with your insurance company for using a diagnosis that is not documented in your records, he is doing you a favor by using the most common complaints by women as your diagnosis.
The other reason for this is that the laboratory that he sent your cultures and PAP smear to MUST have a diagnosis for thier billing also. They will use the "rule out" code which is what the doctor used until or unless a diagnosis is found after the results.
I hope this makes sense to you. Medical billing is a complicated business and your are not the first person to question why the doctor used a diagnosis that they did not have. The bottom line is that insurance companies WILL NOT pay for testing to be done when there is NO PROBLEM. So, a preliminary diagnosis must be used. |
