I'm glad you found a solution to your problem with positioning your hands. For most people, this sort of thing tends to take care of itself, but sometimes more attention to an issue can help, as it did in this case. (A general guideline concerning the most comfortable position of one's hands would not be appropriate since this is an individual matter.)
Thoughts during NSR come from the process of stress release (this is described in the NSR manual). The specific thoughts we have originate with the stress being released. Trying to think specific thoughts during meditation is not recommended because it would tend to complicate the technique, interfere with innocence, interfere with transcending, and create mental strain (which produces stress).
