If you haven't yet seen 28 Days Later, please read no further. It will ruin the experience.
28 Days Later **SPOILER** Of course the movie was meant to be vague on the point of whether or not the entire world, or just England was infected so as to generate thought and speculation. Wrapping everything up in a neat little package is not necessarily the best way to end a movie.
Having said that, here are my thoughts as to whether or not the entire world's population was being wiped out:
The rest of the world was not infected! it was limited to Great Britain
The virus only effects humans and primates. There aren't alot of ocean going primates and the humans obviously couldn't fly a plane when ravaged by the effects of the virus. The rest of the world would establish a quarantine knowing to be on the lookout for people and primates coming from England.
There's plenty of evidence that animals can't be infected with the virus... rats running, Horses herding, Birds flying.. all shots the director deliberately focused on for several seconds.."The rats are running from the infected". Only in Jim's dream did the sheep show signs of possibly being infected... alluding to the fact that animals were not spreading the virus.
Regarding the radio reports about New York and Paris being infected. If you don't want people coming to an uninfected area, tell them the virus is already there! Therefore the rumors were false...disinformation to keep infecteds in UK.
It takes 10 - 20 seconds to show signs of being infected, and it's quite obvious if you ARE infected..so.....it's fairly easy to keep tabs on who is and who isn't infected. It would be far worse if the incubation period were hours or days or even months.
Don't forget that the military commander said "the rest of the world is watching the Simpson's.
Another thing: Hannah says "do you think they saw us THIS time?" This leads us to believe that the plane has been seeing them each time but is just flying over to make sure that they are clean and not infected. Remember, the military commander said that England is under quarantine i.e. the infection is limited to England and no one...NO ONE is allowed to leave.
