In one word - attitude.

In explanation based on my interpretation of Sir...(and in this context, pain and physical hurt go hand-in-hand.)

When pain/hurt is part of our scene-ing, it is pleasurable to both of us. Seeing the fire in His eyes tells me we are both getting off on it. Damage is never caused.

The pain/hurt from punishments is not pleasurable to either one of us. Sir hates having to punish me even though He understands it is a necessity. i HATE receiving it, but understand that i screwed up and have to pay the price. Damage is never caused.

Thus, our attitudes makes the distinction between pleasure and punishment pain/hurt.
