      Family Hx:
      -Begin screening for colorectal cancer with colonoscopy at age 40 or 10 years younger than the age of the youngest affected relative at colorectal cancer diagnosis, whichever comes first
      -If no polyps are noted and the relative was under age 60 at diagnosis, repeat colonoscopy at 5-year intervals, except when a familial syndrome is known or suspected; if the relative was over age 60 at diagnosis, repeat colonoscopy at 10-year intervals as in average-risk individuals

      Personal h/o polyp:
      -obtain colonoscopy in patients with only one or two small (<1 cm) tubular adenomas (in the absence of more significant history) in 5 years
      -Obtain colonoscopy in 3 years in patients with three to ten adenomas or advanced lesions (>1 cm, villous, or high-grade dysplasia) or sooner than 3 years, depending on:
      1.The recommendation of the endoscopist
      2.Whether the polyps were completely removed with certainty
      3.If there are more than ten adenomas
      4.The adequacy of bowel preparation and visualization of the mucosa at the time of exam
      5.Whether the exam was completed
      6.Whether a hereditary syndrome is suspected
      -In patients with large sessile lesions removed in piecemeal fashion, obtain colonoscopy in 2 to 6 months to be sure that there is no residual or recurrent polyp growth
