Cable for 6100/66 dos card

I bought a 6100/66 dos compatible version, but no cables were included. I remember having a weird cable that combined the 45pin mac video output with the weird connector on the DOS board, and then there was a joystick and monitor connector coming off of this cable. I had one in the past. How can I connect a monitor straight to the dos card(assuming I use a 45pin to 15pin adapter for the Mac portion), or alternatively, does anyone have the cable I need?
