Humanity & biological species probably evolved for the most part.
Divine intervention at some point in the past, as evidenced by what ID calls "irreducible complexity" of certain organs & adaptations - bacterial flagellum, wings, eyes etc.
Age & creation of Earth are mostly thought of as scientifically accepted age but with certain aspects being directly created (as per "irreducible complexity" argument).

THEISTIC EVOLUTION:
God created the universe/multiverse and all processes now in existence which drive it.
