They had real bullets (without gunpowder) in the gun so that the bullet tips would show up in the cylinders when the camera looks directly down the barrel of the gun. But when they removed the bullets from the gun, one of the bullet tips broke off and remained in the barrel. For the next shoot they needed blanks for the smoke and sound. When the blanks fired, it forced the broken tip out the barrel and into Brandon's body.
