I'm sure the responsible parties got in trouble. But remember, it was an accident. How do you punish someone for a workplace accident? Yea, they probably got a little careless but it was because of the tight shooting schedule. They didn't mean to hurt anybody. I'm sure film shooting policies were changed because of the incident.
The big thing is that becasue of the accident, hoipefully they have made the film shooting workplace a safer environment.
