I think Frankie was going to kill himself. He went to have one last piece of pie (remember he said earlier after eating a piece of the pie, "now I can go to heaven") before killing himself with the second syringe. Remember that Scrap clearly told us that one syringe was enough to kill someone ten times over. Why would he pack two syringes?

Also, I happen to believe that Frankie's daughter was dead all along (there are many allusions to this) so if that's the case, when Scrap says in the letter "I had hoped he went to you". That's sounds suspiciously like he meant "I hoped he went to be with you in heaven."
