I can only share my recent experience with Belle..

Belle's ears kept getting worse and worse. I tried a couple of different foods and nothing cleared her ears up. The vet had me try Otomax and Keflex which did nothing. Then we tried Baytril Otic, with a special cleaner I was to use daily. That made no difference. In fact they got worse. Nothing was working and we were getting close to seriously discussing ablation surgery if her ear didn't open. (One was worse than the other.)
My last ditch effort suggested by the vet was to try Zymox Otic (you can order online from Amazon) It's the green label without hydrocortisone. I tried that along with Canidae Lamb and Rice. That is the food she was on before they changed the formula.
Thankfully it was the right combination for Belle! Her ears are now clear and she is doing okay on the food. It took a couple of weeks, but I can now see down into her ear.
Can't say that will work for your dog, but that's my story.
I have had success with the Zymox Otic with other rescue dogs too, so it may be worth a try for you.
