It was simply removing a small amount of material from the outer rear corners as needed, sort of knocking the corner off, and blending it in. On most you can't see anything different, or it wasn't needed.

"Is it possible that this configuration was seen on any M4A2 Shermans that were lend-leased to the USSR?"

It is possible.

"Could any of these turrets have had loader's hatches installed?"

Yes, in fact they all did.
