PS2 making grinding noise and ruining disks

My PS2 has been making a sort of grinding and slipping noise for a few months now. Sometimes simply restarting would correct the problem but now it just doesn't seem to want to read disks. Also, I have noticed that it its making small scuff/scratch marks on the surface of my disks (games and DVDs) especially dark surfaced ones. When the disk plays you can hear the laser gear moving - its loud. Is the laser misaligned or is the gear toast? Help, please!
