I have a samll querry here, please if you advise on the same.
Would the detailed reference letter mentioning my dotnet skill given in DEC 2005 by my Manager and a HR style experience letter given when I left the company (start date: Jan2004 to date: Feb2008) be enough for the same company to prove that I had worked on dotnet during my whole tenure with the company.? (for MODL: it is 12 months out of recent 4 years).

In any case you will be required a reference letter mentioning your job was part time or full time and job start date and end date. In the same letter you can ask them to add technology you worked. If you can't get another reference letter from your past company, you need to submit statuary declaration for the same. It is as per my understanding of the complete process

Is there a form or format of statuary declaration can be refered to?

following is the format of statutory declaration form, fill it according to your details and make it notary on Rs.10/- stamp paper. wish u good luck
