This is the basic problem for many if not almost all Tour players. The problem is mostly visualizing the last two or three feet of the putt plus tempo control. You are probably hitting the ball a wee bit, and this puts too much "send" on the ball and causes lip outs. Seeing the last two or three feet vividly with the ball rolling through this segment with slowing speed to a nice drop speed over the edge will help calm your tempo down a bit, so the delivery of the ball with more controlled and slower speed helps you sink these putts.
