BCBS of MA rejection- need to write a letter

BCBS of MA rejected my application even though all the results I submitted meet their requirements. FSH on Day 3 is 10, and it is 12.9 on day 10 after CCCT. They made decision based on the test I did back on 10/23/10.My FSH level was 23.6 on Day 3 at that time. I am 40 years old this month. My plan is BCBS PPO. I never do IVF before. I am going to appeal. How to write appeal letter? Please help me.
