Welcome to the boards Jean! I hope you are able to find the information you are looking for on here. I can help you with the testing and timing, but I've never used an agency so I can't help you there.

Tests:
Infectious disease ie: HIV, Rubella, Chicken Pox Vaccine etc...
Sperm Analysis (may want to ask for karyotyping as well)
HSG - hystosonogram
Some clinics like to do a mock cycle (highly recommend doing this, that way you avoid any lining issues that can arise during the real cycle)

Timing:
You can anticipate anywhere from 3-6 months to get a cycle going. It really depends on if you are going with a clinic that you are already with or a new clinic. 6 months if it is a new clinic and you have to do all the testing and the mock.
