I have twin boys from my second DE IVF cycle at the Clinic for Reproductive Medicine and Gynecology in Zlin in the Czech Republic. There are several other graduates of that clinic on these boards, as well as its sister clinic, Reprofit, in Brno, and some other Czech clinics, too.
Of course, if your wife does need a hysterectomy, then you'll need to turn to surrogacy or adoption, but I don't think (know there are many veterans of ectopics here, so maybe they'll chime in) that's the norm for a tubal pregnancy, unless there are other complications. I'm less enthusiastic about the Czech Republic these days, not because I didn't have a good experience, but because rising clinic rates and rising airfares are making it less attractive financially. Once you add in the hassle of international travel, language barriers, and finding a local physician to monitor you, the lower cost U.S. options start to look better (although the tourist experience in the Czech Republic is wonderful).
Had my second Czech cycle failed, I planned to go to CNY in the Finger Lakes region of NY. Shady Grove in the D.C. suburbs also has low cost split DE cycles.
