can you reproduce it then in a lower gear? I'm presuming that the top gear fail was at full throttle - perhaps a blat up the dual carriageway in 4th would reproduce it?
It might also be when your throttle position isn't matched to engine speed - i.e. when you're pumping more fuel in than it wants for that particular rpm - indicative of overgearing (although it sounds a little extreme to be that)
