Just saw your posts to CG further down the page. It is great you have these doctor's records!! One suggestion on documentation would be to try to create a timeline, focusing on the custody schedule and the doctor's visits. I would get a blank calendar, and mark in all the dates your ex had custody of D. Then, mark in the days that you took D to the doctor. Other things like discipline issues at school would be good to add in, too. I think this would paint a pretty convincing picture of what's been going on here... the timing of bio-dad custody relative to the medical issues is really key.
