When I did my search I turned up a copy immediately and now it seems they are hard to find.
THE DELTOID LIFTS
THE A-BOMB OF THE BERTHOLY-METHOD
Warning from your Golf Professional, Paul Bertholy: Failure to utilize this near miraculous exercise is detrimental to the health of one's Golfswing.
The position is bent over with you back horizontal and supported on 1 arm on a chair or bed. With the other arm palm down grab the weight ('Bertholy Swing Pipe' ideally) and let you arm hang verticall. Raise your arm laterally to horizontal with your palm down.
Bertholy's record (at age 65) was 1750 consecutive 10lb lifts. He suggests you start with sets of 20 and 100 total a day for the first 2 months. He says when you can handle 1000 a day you will likely add 20-40 yards to your drives even at age 65.
For the Kinesthetic procedure I think he intends you to be in 6/100s with the stick horizontal and parallel to your feet. THe back of your lead and the palm of your trail hand should face away from you. When I do it I take a very mini backswing and then take the downswing with my lead arm while resisting with my trail. Then I just let go of the trail. The only thing I felt comfortable hitting was the bed. I then did this with no surface to hit (I was cheating a bit with Program #3 - Golden Excercise which is the first one done with a club).
BTW - Be very wary of doing this with a club and no surface to hit. It puts a fair strain on your lead wrist.
