In one of the excercises, Bertholy has you practice moving your lead knee to start the downswing and not allowing anything else to move. When you try this you will find it difficult to stop other movement and accomplishing this is likely done by limiting the amount of knee movement.
The knee movement for me starts a hip rotation/shift. THe shift then starts a drop (as I don't allow my view to change, my head stays in place). In a real swing it only takes an instant of this BEFORE shoulder rotation to insure an inside path.
