Usually lack or no evidence of relationship, if you have not meet the individual in person, if you have filed for many k1 visas, errors or imcomplete documentations (although they do allow you to fix errors and to send the required documentation needed).
There might be other factors, but as long as you have an establish, legitimate relationship, all required documentation and requirements, you should get the approval.
