A couple of soundboard shows from Reality '04 have it. Proper recordings that have been 'released', I'd guess the BBC Glastonbury [2000] audio from 6 Music?

The last and the best last recording of S2S is on a boot called Let's Go, I have it and it's brilliant from 2004.

I know he played it at the Borgata in Atlantic City which I was at. There are some boots of the show. I am not sure if that was the last time. Take a look at setlist.fm.

I believe it was performed at his final live concert at the hurricane festival 25.6.04.

I haven't seen any newer version published, but I guess there might be something in a vault somewhere.
For instance, the Hurricane performance was filmed by a TV crew (from Arte), but I have only seen two songs published (Heroes and All the young dudes).
Come to think of it, Glastonbury 2000 was at least partially filmed also, but I've never seen footage of STS, though the radio broadcast sound is available.
